We are an NDIS registered provider based in Newcastle NSW. We have great connections with local services that have been built through a lifetime of living and working in Newcastle.
Focused on support coordination, specialist support coordination and psychosocial recovery coaching.
We can provide services for anyone with NDIS funding aged 7+
Works with you to make sure you understand what is happening with your NDIS plan and how you can use the various kinds of funding within it.
Helps you to make the most of your NDIS plan and achieve the goals you have set for yourself.
Connects you with the services you choose based on your needs.
This is a higher level of support coordination. The focus of this service is on supporting participants with high needs to work through extremely difficult and complicated situations. Examples of this could be a combination of homelessness, complex mental health issues, a transition from a long term stay at an inpatient mental health facility back in to the community, or a transition from jail back in to the community.
A recovery coach is an NDIS funded worker that has mental health knowledge. A recovery coach will:
Spend time with you, and people important to you, to get to know you and understand your needs.
Help you to find out about different services and supports, and how these can help you.
Help you get support from mental health services.
Share knowledge about the NDIS, support you with plan reviews, and managing services you are connected to.
